Ziggeo Adds New Form Template

Ziggeo has just added a new template to its open-source submission forms. The new form is a one-pager that asks for basic information and a video recorder all on one page.

Here's what the form looks like:

This form supplements another form template in which basic information is first submitted before a video recording is requested.
